CANCELLATIONS & RESCHEDULES

Appointments must be cancelled or rescheduled 24 hours prior to your appointment time. â€‹Failure to do this will result in a charge of 50% your service cost.

NO-SHOWS

Cancellations or reschedules within 30 minutes of your scheduled appointment time, as well as being 15 or more minutes late to your scheduled appointment time qualifies as a no-show. You will be charged 75% of your total service cost.

RUNNING LATE TO YOUR APPOINTMENT

We understand being a few minutes late! We ask that you text us with a heads up if possible. If we have not heard from you 5 minutes after your scheduled appointment time, we may send you a text.

After 10 minutes of no response we may call you.

We will accept late appointments up to 15 minutes past your scheduled time; however, this will take time away from your appointment. Anything past 15 minutes qualifies as a no-show, even if you arrive. 

REFUNDS AND RETURNS

All sales are final. We do not offer refunds returns on services or product purchases. If you are unhappy with a product or service you received from us, please reach out and we can figure something out! â€‹

 

SHIPPING

We only ship within the U.S.. All sales are final. You may not add/remove items once an order is placed. If you are placing an order to be shipped through our acne portal, this will not ship until payment is received. Not all orders are shipped directly from us, some will be sent through the according product company. 

GUEST POLICY

Guests may only accompany your appointments if you give us a heads up and is only allowed on a case-by-case basis. We do not allow small children as guests. Clients under 18 may have a legal guardian accompany their appointments. In the case that someone must accompany you, we have a waiting area outside of our suite, as well as a front lobby inside Entrance A. 

CARDS ON FILE

All appointments require a card to be held on file. We reserve the right to charge said card in the case of no shows and last minute cancellations or reschedules breaking our policies. In the case that a card on file declines, an invoice will be sent to collect payment. Failure to resolve said invoice within 24 hours waives your ability to rebook any future appointments. 

CANCELLATIONS ON OUR END

We reserve the right to decline service for any reason where we believe the client is not a suitable candidate. If this is the case, we will reach out to you immediately after reviewing your intake form or at the time the contraindication presents itself. Client safety and providing ethical service is our top priority.

Cancellations due to weather: In general, if schools close, so do we. This applies to the Appleton, Kimberly, and Oshkosh school districts. We will reach out as soon as possible to inform you of closure and to reschedule your appointment. Late cancellations or reschedules will not be charged any fees in this case.

Cancellations due to sickness or family emergency: We will reach out to you as soon as possible to inform you of closure and to reschedule your appointment. Since we do not charge you a fee for late cancellations due to uncontrollable circumstances like these; you will not receive compensation if we reschedule you due to similar circumstances. 

INTAKE FORMS

You agree to all statements made on intake forms when you sign. A completed and signed intake form is required for all appointments. You must fill out the intake form truthfully and to the best of your knowledge. Intake forms can be found here or may be sent to your provided phone number via text. Changes may not be made to intake forms. In the case that a change needs to be made, you must complete an additional form. We will always base treatment and product usage off your most recent intake form completed. 

 

For the Acne Bootcamp Program Onboarding appointment, your intake form must be completed 24-48 hours ahead of time. If this is not done, your appointment will be cancelled and will need to be rescheduled. 

PHOTOS, VIDEOS, AND AUDIO RECORDINGS

All media posted, published, or used in any way outside your files are agreed to be taken and used within the appointment's corresponding intake form agreement. 

MINORS

Clients who are under 18 may receive select services with us. A release form is required to be completed by the minor's legal guardian prior to the scheduled appointment time. Failure to submit this by the time of the appointment will result in said appointment being cancelled and treated as a no-show. A legal guardian is not required to accompany said minor to the appointment but may if they choose to. All intake forms sent to a client who is a minor must be completed with a legal guardian and signed by both the minor and legal guardian.
